Amyloid-beta is an abnormal protein considered a major cause of Alzheimer's-type dementia.
In recent years, several new drugs that reduce amyloid-beta accumulated in the brain have been approved in Japan one after another.
These are relatively expensive drugs classified as antibody pharmaceuticals.
In clinical trials, these new drugs were expected to be highly effective, as they were shown to significantly reduce the amount of amyloid-beta in the brain and statistically significantly improve the "score" that quantifies the degree of cognitive function.
However, according to meta-analyses of the results of multiple clinical trials, it was found that even if the reduction of accumulated amyloid-beta and cognitive function scores improved significantly, patients and their families felt little improvement in symptoms or quality of life (QOL).
In other words, even if the results showed a statistically significant improvement in numerical values compared to the control group (the group administered a placebo with no effect), the extent of that improvement did not reach the threshold (called the "minimal clinically important difference") that patients or their families could perceive as "getting better."
If the people involved cannot feel the benefits of the treatment, it has come to be thought that using expensive therapeutic drugs might be extremely poor value for money.
Imagine you have become a patient. For example, consider this situation:
You "Doctor, about my current medication, I don't really feel like it's working... Could you switch me to a different one?"
Doctor "No, no, that's not the case. The numbers are definitely getting better."
You "But I still feel just as unwell, and I have no appetite. I don't feel like I'm getting better..."
Doctor "That's just because you aren't feeling it, but the test results are definitely improving. Please rest assured."
It seems to be a drug with low patient satisfaction.
This can hardly be said to be beneficial to you.
Clinical medicine is only meaningful when it benefits the patient.
In this way, it is better to consider that having a statistically significant difference in some numerical value and having clinical significance are completely different things.
In many clinical studies, fluctuations in "numerical values" are examined.
And in the papers, they proudly proclaim the conclusion that the values have "improved" significantly.
As if highly significant results had been obtained.
However, when it comes to whether it is a clinically meaningful improvement, there are surprisingly many clinical research papers that do not focus on that.
Scientific papers do not become "scientific evidence" just by being published.
There are many cases where, as a result of verification by others, reproducibility cannot be confirmed or clinical utility is not recognized.
This is precisely why Dr. Tasuku Honjo says, "90% of papers are lies. Only 10% will remain after 10 years."
There are countless advertisements and labels for "Foods for Specified Health Uses (FOSHU)" and "Foods with Function Claims" that claim there was a "statistically significant difference."
However, what those words mean is not that it is "clinically significant."
And in many cases, even the confirmation of reproducibility has not been done.
It does not necessarily serve as evidence that those products will improve diseases.
Please be very careful about "statistically significant differences".
And never, ever think that you want to cure a disease with health foods or supplements instead of taking medicine.
