[Background/Objective] Over the past decade, physical activity (PA) (or "exercise") has been identified as one of the main modifiable factors influencing the progression of Alzheimer’s disease (AD) pathology. We conducted an umbrella review to summarize the evidence regarding the association between PA/exercise and the risk of developing AD, as well as the effects of exercise interventions on the progression of AD.

[Results] 21 studies were included. The results with the strongest evidence revealed a positive effect of PA on AD risk. Specifically, meeting WHO recommendations for PA was associated with a reduced risk of AD. It was also revealed that exercise has positive effects on cognitive function, physical capacity, and functional independence.

[Conclusion] There is strong evidence that regular exercise prevents AD risk, but the dose-response relationship remains unclear. While exercise appears to improve several aspects of AD patients, further research is needed to clarify the exercise characteristics that promote maximum benefits.
