Can Orexin Be Regulated? How to Stabilize Sleep and Attack Rhythms | Textbook of Cluster Headache #08
What is orexin and what does it do?
Orexin (hypocretin) is a neuropeptide secreted from the hypothalamus of the brain.
Its main role is to maintain a state of wakefulness.
・Maintaining consciousness while awake
・Controlling the switch between sleep and wakefulness
・Creating rhythms in conjunction with the body clock
When this orexin is disrupted, sleep quality decreases and the wakefulness rhythm collapses.
In cluster headaches, this disruption in rhythm may be affecting the timing of attacks.
“Regulating” it is difficult—to be honest
First, let me be honest with you.
There is currently no self-care method that directly controls orexin.
However, orexin is deeply linked to your daily rhythm.
In other words,
👉 Regulating your daily rhythm leads to stabilizing your orexin rhythm
This is the realistic approach.
Things you can do with your lifestyle
① Fix your wake-up time
The orexin rhythm starts moving triggered by “waking up”.
・Wake up at the same time every day (within ±1 hour even on holidays)
・Open the curtains when you wake up, and go outside even for 5–10 minutes
Fixing your “wake-up time” is more important than your “bedtime”.
② Move your body during the day
Orexin is also stabilized by your activity level.
A walk is fine. The contrast of “active days and restful nights” leads to a stable rhythm.
③ Reduce stimulation at night
Strong light from smartphones and brain stimulation can trigger orexin at night, disrupting your sleep rhythm. Avoid smartphones one hour before bed and keep the room dark. It is important to be conscious of "turning off your wake-up mode" at night.
④ Keep naps under 20 minutes
Long naps disrupt the orexin rhythm. If you are really sleepy, limit it to 20 minutes and avoid napping after the afternoon.
Medical Approaches (Reference)
In addition to lifestyle habits, here are some things that may be considered at a doctor's discretion.
Melatonin
A hormone that regulates the biological clock, it is used in some cases as a preventive medication for cluster headaches.
"This article is for informational purposes only and is not a substitute for diagnosis or treatment"
Orexin receptor antagonists
A type of sleep medication that adjusts the function of orexin. There are reports suggesting it may reduce nighttime attacks, but its use for cluster headaches is not established as a standard treatment. Consulting a specialist is a prerequisite if you are considering its use.
Summary
There is currently no way to "directly regulate" orexin.
However, by stabilizing your daily rhythm, it becomes easier to regulate your orexin rhythm as well.
What you can do is simple.
・Wake up at the same time
・Get sunlight in the morning
・Stay active during the day
・Reduce stimulation at night
You don't need to do it perfectly.
"Not letting it get too out of balance" is enough to make your body respond.
