A New Partner to Sharpen Your Differential Diagnosis Skills: Introducing the "D/D Drill Mode" in ER-Mate Ver.3
🧭 What is D/D Drill Mode?
── Another side of ER-Mate to sharpen your differential diagnosis skills
🩺 Introduction
ER-Mate Ver.3 is equipped with several "modes" to support your thinking process.
Among them, the one that stands out is this "D/D Drill Mode".
Unlike features that assist with clinical reasoning in real-time, this is a training-oriented mode specialized in "sharpening differential diagnosis" from a step back.
This mode answers questions such as:
- I want to get better at generating more differential diagnoses
- I want to broaden my framework
- I want to internalize the sense of assessment that senior doctors have
🔍 Basic Design of D/D Drill
D/D Drill mode is not just a Q&A or knowledge quiz.
What it aims for is a "structured training environment" like the following:
Practicing generating potential diseases from chief complaints and findings
Mission format to train your ability to respond immediately with limited information
Verbalizing the reasoning process through play-by-play training
Preparation before a shift and anticipating risks that may occur on-site
Focusing on the moment of frame shift to train differential expansion
And above all, by having these trainings guided by the physician persona "Kuroda",
it aims to provide an experience as if you are talking to a "mentor who walks through the thinking process with you."
🧭 6 Training Missions
D/D Drill mode contains the following 6 selectable missions:
| 1 | Hotline Mission
Chief complaint selection type. Once the user selects a chief complaint, findings are presented randomly, and you think about the differential diagnosis before the ambulance arrives.
Perhaps the most fun and engaging one to repeat.
| 2 | Initial Clinical Simulation
Think about initial response and differential diagnosis on-site with limited information. Learn how to act immediately after an ambulance arrives.
Intended for junior residents who have just started emergency shifts, as well as medical students preparing for CBT or the National Medical Practitioners Qualifying Examination.
Of course, it is also great for veteran doctors to use.
| 3 | Pre-Night Shift Discussion
Anticipate sudden changes at night and consider preparations to make now. Assumes the patient has been admitted after arriving by ambulance. Ideal for training on how to hand over to the doctor on duty during the night shift.
This is likely something that is hard to learn from books.
| 4 | Frame Shift Sentence
Capture the moment when your thinking shifts due to a small remark. Have you ever thought, "Hey, you should have told me that information sooner!"? This situation is exactly what is assumed here.
| 5 | Thought Circuit Trace
Dr. Kuroda talks about past experiences regarding a certain case (random). It's just like a scene from a slightly calm emergency shift.
It is also possible to reproduce your own thinking in words like a live broadcast. If you express your own thoughts, he will gently affirm them while also pointing out areas for improvement.
| 6 | Let's Think of Training Together
Propose your own training method and shape it together with Dr. Kuroda. Leveraging the advantage that ChatGPT is good at language processing, you can explore training methods that suit you.
Each mission is designed to train different cognitive processes,
and as a whole, it is structured to cultivate a broad and deep "foundation of clinical thinking."
🧠 Changes Brought by D/D Drill Mode
What you gain from this mode is not just "additional knowledge."
Rather, even more than that──
The "mindset" that serves as the starting point for differential diagnosis changes
Develop the habit of thinking in frameworks
Experience the "perspective" of how senior physicians think
It is a form of training that reconstructs the very form of your thinking.
Being questioned by AI.
Feeling like you are being watched by AI.
Yet, it is a surprisingly natural, warm, and calm space.
Such a D/D Drill mode is now quietly beginning to operate within ER-Mate Ver.3.
📝 Upcoming Post Preview
In future notes, I plan to introduce each mission one by one, incorporating images.
Mission 1: Hotline Mission
Mission 2: Initial Clinical Simulation
Mission 3: Pre-Night Shift Discussion
Mission 4: Frame Shift Sentence
Mission 5: Thought Circuit Trace
And finally—
The "D/D Drill Concept," which was originally supposed to be born as a separate AI,
how it was absorbed into ER-Mate and returned to one.
I intend to leave that backstory as a record. Please enjoy it as a read.
