Regular hours are until 5:00 PM. But, I didn't finish work until nearly 9:00 PM.
Today, I think I'll write a little bit about what it's like on the front lines.
Regular hours are until 5:00 PM.
But when I realized it, the clock was nearly 9:00 PM.
My lunch break was also mostly work.
I've finally finished today's tasks.
This week, there were four new cases.
For three of them, I handled everything from the initial contract to creating the care plan all at once.
On top of that, there was a user returning to service.
After being hospitalized, their condition declined, so I had to redo their care plan.
For one person,
there was a service provider meeting the day after the contract.
It was quite a tight week.
Being a care manager,
is also a job of writing records.
Assessments
Care plans
Progress notes
Service coordination
Everything must be documented.
But sometimes I wonder.
Sometimes, even though I've written it down properly, it doesn't get read.
I hear voices from the attached office.
What's the status of this?
Could you check on this matter?
When I look at the records,
it is written down properly.
So I answer.
But then, the same inquiry comes again.
I am leaving it in the records.
I am also handling the requests.
Even so, the same issue keeps coming back.
Every time that happens, I wonder.
Whose time is this,
really for?
Time for the users.
Time for their families.
That is where I really want to spend it.
Care managers are
said to be coordinators.
Between medical care and nursing care.
Between the office and the family.
Between the system and the front lines.
Connecting all sorts of things.
But sometimes I wonder.
Rather than a coordinator,
have I become someone who just fills in all the gaps?
Even so,
I will head out to the front lines again tomorrow.
So that someone's life,
can continue, even if just a little.
Thinking about things like that,
I closed today's records.
I didn't know about the accident on the way home, so the crowded train took twice as long. It was a day that wouldn't end, and it became the next day.
Because of that, my comments and follows are delayed. I am very sorry.
