Significant Expansion of Intuitive Surgical Instrument Usage + Introduction of Endoscopic Surgical Support Equipment Fee - How Will the Profit Structure of da Vinci 5 Change?
What happened?
Entering 2026, there have been two major changes in the economic environment surrounding the da Vinci 5. The first is the
expansion of the number of uses for Intuitive Surgical's Force Feedback (FF) instruments (announced May 21, 2026).
The second is the establishment of a new fee for endoscopic surgical support equipment (effective June 1, 2026). The former reduces costs, while the latter increases revenue. Both act to significantly accelerate the break-even point for the da Vinci 5. In this article, we will explain these two pieces of news and share the updates made to the DV5 break-even simulator.
News Analysis 1: Significant Expansion of FFB Instrument Usage
What has changed?
On May 21, 2026, Intuitive Surgical announced a series of innovations aimed at advancing the "Quintuple Aim."
Among them, the most notable point is that the number of uses for da Vinci 5 Force Feedback instruments has been significantly increased from 6 to up to 15.
"Five of the six available Force Feedback instruments have had their number of uses increased from six to 15 (one instrument is cleared for 10 uses)."
Note that the official announcement does not specify which product is set to 10 uses.
Furthermore, it has been stated that an increase in the number of uses for core instruments (compatible with both Xi/X) is planned for 2027.
"In 2027, we anticipate that the number of uses on certain core instruments will again increase, reducing per-procedure costs for da Vinci surgery and supporting the Quintuple Aim. This Extended Use Program reflects years of investment, design improvements and rigorous testing, resulting in greater reliability and extended use without compromising precision, safety, or reliability."
How important is it: Direct impact on cost per procedure
In the operating costs of robot-assisted surgery, instrument costs are the largest variable expense item. An increase in the number of uses means that the depreciation cost per use decreases.
Let's calculate it specifically. For example, in the case of the FFB Cadier Forceps (simulator reference price: approximately 166,800 yen):

Approximately 16,700 yen per unit, a cost reduction of about 60%. When using 3 to 4 instruments in a single procedure, a cost reduction of approximately 50,000 to 70,000 yen per procedure can be expected from FFB instruments alone.
Background: Why was the low usage limit a problem?
da Vinci 5 received U.S.FDA 510(k) clearance in March 2024, and received regulatory approval from the Ministry of Health, Labour and Welfare in Japan in February 2024.
Since its launch, the low usage limit (6 uses) of Force Feedback instruments has been a major cost concern for adopting facilities. While core instruments for Xi/X can be used 14–18 times, FFB instruments could only be used 6 times, making the cost per use relatively high.
FFB Instrument Price Range
FFB instruments are not in the same price range as core instruments, but in a premium price range. The following is a comparison in the U.S. market.

* Since the official list price of FFB instruments is not public, prices from the secondary market (Synergy Surgical) are used as reference values. As this is not a direct comparison with official prices, the actual price difference may be smaller.
However, with the usage limit expanded to 15 times, the cost difference per use has narrowed significantly.
Considering the added value of the Force Feedback tactile feedback function, the cost hurdle is now considerably lower than before.
Furthermore, Intuitive Surgical previously launched the "Extended Use Program" for da Vinci X/Xi in 2020 and has a track record of expanding usage limits for core instruments. This latest expansion is considered an extension of that initiative.
News Commentary 2: Establishment of the Endoscopic Surgical Support Device Add-on Fee (Effective June 2026)
What has changed?
In the 2026 fiscal year (Reiwa 8) medical fee revision, an "Endoscopic Surgical Support Device Add-on Fee" was newly established. It has been in effect since June 1, 2026.

Direct Impact on Revenue
If a facility with 200 or more cases per year can bill for the add-on, an additional revenue of 150,000 yen per case can be obtained.
For example, in a facility with 300 cases per year, where 250 cases are eligible for the add-on procedure:
250 cases × 150,000 yen = 37.5 million yen in additional annual revenue
This amount is comparable to the annual lease fee for a da Vinci 5 and has a significant impact on the break-even point.
Beware of the "Asymmetric Trap" in Prostate Cancer Surgery
There is an important point to note here.
・Count target for 200 cases per year: Including prostate cancer surgery (K843-4) (26 categories)
・Eligible for add-on billing: Excluding prostate cancer surgery (25 categories)
In other words, while prostate cancer surgery can be used as the "numerator" to clear the 200-case hurdle, it is an asymmetric structure where the add-on cannot be billed for prostate cancer surgery itself.
In facilities where prostate cancer surgery accounts for the majority of cases, there is a possibility that the number of cases eligible for the add-on will be limited, even if the total exceeds 200 cases.

・Urology: Renal malignancy surgery (partial/total nephrectomy), bladder malignancy surgery (total cystectomy), pyeloplasty
・Gastrointestinal surgery: Gastrectomy/total gastrectomy (malignant), colon malignancy resection, rectal resection/amputation, esophageal malignancy surgery
・Gynecology: Uterine malignancy surgery
・Thoracic surgery: Pulmonary malignancy surgery (segmental/lobectomy), mediastinal malignancy surgery
・Cardiac surgery: Thoracoscopic valvuloplasty (K554-2)
Updates to the da Vinci 5 Break-even Point Simulator
Following this announcement, we have made the following updates to the "da Vinci 5 Break-even Point Simulator".
1. Addition of Endoscopic Surgical Support Equipment Add-on Toggle (NEW)
We have added an Add-on ON/OFF toggle to the settings panel. When turned ON:
200-case count: Real-time display of whether the total number of cases for selected procedures (including prostate cancer) is 200 or more
Number of add-on eligible cases: Automatic determination of the number of cases eligible for the add-on
Annual add-on revenue: Instant confirmation of additional revenue from the add-on
Procedure table: Displays "+Add-on" mark and "+150k" for eligible procedures
The default is OFF (as many facilities have fewer than 200 cases per year). Please turn it ON according to your facility's situation.
2. Update to points for rectal resection/amputation (NEW)
With the 2026 revision, K740-3 was newly established, and the points for robot-assisted rectal resection/amputation were increased from 83,930 to 87,430 points (+3,500 points). The point data in the simulator has been updated.
3. Update to FFB instrument usage count (NEW)
Updated 5 types from 6 uses to 15 uses, and 1 type from 6 uses to 10 uses.
4. Reflection of 2027 core instrument expansion (NEW)
Updated Xi/X compatible instruments (10-use products) to 15 uses. Since the specific number of uses is not explicitly stated in the official announcement, we have adopted 15 uses as an estimated value for the simulation.
5. Addition of FFB usage rate slider (NEW)
*Default value: Added a slider adjustable from 0% to 100%, with 30% as the default. The average I&A unit price is automatically calculated based on the usage ratio of FFB and core (Xi/X compatible) instruments.

The combined effect of these two changes on the break-even point
These two changes act in the same direction regarding the break-even point.
