On Thursday, September 18, 2025, from 19:00 to 20:00, I served as a lecturer at the FY2025 Chiba Prefecture Home Medical Care Startup Support Project (Home Physician Training Program) titled "Chiba Home Medical Care Introduction".

This is an online training series aimed at doctors and medical professionals who want to learn about home medical care, sharing practical examples of community-based integrated care and multidisciplinary collaboration.
Lecture Theme
"Are you forgetting about Geriatric Health Services Facilities (Roken) within the Community-based Integrated Care System?"
Focusing on the role of "Geriatric Health Services Facilities (Roken)," which is often overlooked in community-based integrated care, I spoke about topics including: ・Collaboration models born in the field of Roken, which sits "between" medical care and nursing care ・The potential of Roken in ACP (Advance Care Planning) and support for returning home ・Implementation examples of information sharing and multidisciplinary collaboration in the community-based integrated care system, incorporating actual data and anecdotes from the field.
Alongside a diverse lineup of lecturers
This "Introduction" series is composed of a highly accomplished group of lecturers who are practitioners from both within and outside the prefecture.
Tsuboya also greatly enjoyed the other lecturers' sessions and gained a lot of insight.
