🩺 Care Management for Individuals with Heart Disease

We held a training session where three care managers deepened their knowledge regarding support for users with heart disease.📚✨

Heart disease is a condition that progresses gradually while repeating cycles of recurrence and re-hospitalization.💓 Because conditions can worsen suddenly or improve through treatment, it is essential to engage in ways that do not overlook daily changes.👀

In the training, we learned focusing on the following points:🖊️

✅ The importance of supporting daily self-management, such as medication management, weight, salt, and fluid intake management.💊⚖️💧
✅ The importance of multidisciplinary collaboration among doctors, nurses, rehabilitation staff, and others.🤝
✅ Support for maintaining functional abilities and social participation that is true to the individual.🏡
✅ The necessity of advance care planning (ACP) when heart failure progresses.📝

The participating care managers also shared opinions based on their respective field experiences.🗣️

💧 Nakai pointed out how to interact with individuals who have fluid restrictions, and noted cases where doctor's instructions regarding fluid intake for the elderly are ambiguous. Additionally, touching on the negative aspects of over-providing services, the opinion was raised that "as a care manager, it is important to have a perspective on utilizing long-term care insurance appropriately."⚠️

🤝 Konno reaffirmed the importance of collaboration with nurses and rehabilitation staff.

🏠 Sasaki shared opinions on the sense of security gained by having home-visit services in place, and the importance of daily observation for elderly individuals living alone.👁️

Heart disease is a condition that requires support that addresses not only physical aspects but also the anxieties of the individual and their family.🫶 We would like to apply what we learned this time to our daily care management work.💪
