Medical Insurance Centered on Health Insurance - Simplified Version

In this note, I organize topics that examinees often stumble over by using a method of "understanding the reasons rather than rote memorization."

Have you ever had your mind go blank when questions about late-stage elderly medical care or seamen's insurance appeared on a mock exam?
"Where in the textbook was this...?"
This field is where examinees tend to stumble the most. However, when you organize the problems, one single tip makes everything much clearer. Today, I will share just that one tip with you.
The tip is just this
Is the question about the "content of medical care" or the "framework of the system"? First, distinguish between these.
・If it is about the content of medical care → You can solve it using the Health Insurance Act
・If it is about the framework of the system → You can solve it using the specific knowledge of that law
Why can you solve it this way? Let's look at it in order.
Why can "content" be solved with the Health Insurance Act?
The content of medical care refers to parts like these.
・What kind of treatment can be received
・What rules hospitals follow for medical examinations
・How much to pay for that medical care
If this were different for people in health insurance, national health insurance, or late-stage elderly medical care, patients would be in trouble, right? It would be a disaster if the medical care you could receive for the same illness differed depending on your insurance.
That is why the government clearly writes in each law, "Use the rules of the Health Insurance Act for the content of medical care".
For example, if you are asked in late-stage elderly medical care, "How are hospital treatment rules and medical fees determined?" The answer is "the same as the Health Insurance Act." It is written that way in both the National Health Insurance Act and the Act on Assurance of Medical Care for Elderly People.
Therefore, if you study the Health Insurance Act thoroughly, you can generally solve questions about the content of medical care even if you are seeing the provisions for the first time.

Why can't the "framework" be solved with the Health Insurance Act?
On the other hand, the framework of the system varies for each law.
・Who operates it
・Who is covered
・How insurance premiums are collected
For example, if you are asked, "Who is the insurer for late-stage elderly medical care?" The answer is "the Wide-Area Union." This does not appear in health insurance. It is a unique mechanism specific only to late-stage elderly medical care.
If you try to solve problems regarding this framework using the logic of Health Insurance, you will get them wrong. You have to memorize these for each specific law.
There are only two things to memorize.
・Content issues (rules for medical care/treatment, prices) → Can be solved using the Health Insurance Act
・Framework issues (operators, eligible people, insurance premiums) → Individual knowledge of that specific law
If you encounter a problem you have never seen before, first ask yourself, "Is this about content or the framework?". Just remembering this one phrase will keep you from getting stuck.
Just one warning about a common trap.
Do not think, "So, Late-Stage Elderly Healthcare is just a part of Health Insurance."
Late-Stage Elderly Healthcare is an independent system. When you turn 75, you withdraw from your previous Health Insurance or National Health Insurance and transfer to this one.
The only things aligned with Health Insurance are parts like the "rules for medical care content." The system itself is separate. It is safer to keep these distinct in your mind.

・Late-Stage Elderly Healthcare is an independent system
If you can say just this much, you will be able to read unseen problems in this field much more calmly. It is okay to stop here.
If you have the capacity, just one more thing.
Seamen's Insurance is the system closest to Health Insurance among these three. Since it was originally intended to cover the Health Insurance portion for company employees, much of its benefit structure follows that of Health Insurance.
On top of that, there are unique benefits specific to maritime work. For example, the missing person allowance. This is a benefit for when someone goes missing at sea, which does not exist in Health Insurance.
It is closest to Health Insurance, but has unique additions for maritime work. That is a good way to think of Seamen's Insurance.
